Rental Forms

Here you will find the required forms for renting medical equipment.
Every rental will require 2 forms at least. The required forms need to be signed and completed prior to the rental period.

1. Rental Agreement form - states the terms and conditions of the rental.
2. Payment authorization form - Needs to include the billing address and delivery address (if the equipment will be delivered to you)
